The order form prints exactly what you submit, and paid orders cannot be changed. That single rule is why the form deserves ten minutes of care: every field becomes part of the card, and a mistake after payment means a new order, not an edit. This walkthrough goes field by field so the first submission is the right one.
Before you open the form
Have two files ready before you start: the photo and, if you want your own, the signature. The photo rules are strict because the card is built from that file, and the full checklist is in the photo tips guide. The signature is optional, and the exact pen and paper rules are in the signature guide. Preparing both first means you are not rushing the form with a bad file attached.
The state and name fields
Pick the state first, because it decides the template. A practical rule from the FAQ page: order a neighbouring state rather than your own, since door staff know their home state's card by heart. Then enter your name exactly as you want it printed, because the card does not get a second proofread.
Date of birth and the 21+ rule
US state cards are 21 and over. A younger date of birth is automatically corrected to show over 21, and under-18 details are never printed. Under-21 formats exist only for UK and Canadian cards, which is covered in the under-21 guide. Enter a date you will remember, because the card and its duplicate will both carry it.
The photo upload
The photo is the single biggest factor in how the finished card looks. Use a high-resolution file with even lighting, a plain, ideally white background, neat hair and no glasses. If the file looks blurry on your screen, it will look worse on the card, so retake it before uploading.
The signature upload
The signature field is optional. Leave it blank and a clean signature is generated for you. If you send one, it must be a black Sharpie Fine Point on plain white paper, signed at about 5 cm wide and photographed flat from above. The wrong pen gets orders sent back, so follow the signature guide or skip the field entirely.
The extra info box
This is where a custom address goes. Type the address you want printed on the card into the extra info box at checkout. Leave it blank and a random, real address is printed instead. The box only affects the card, not the shipping label, and the difference is explained in the custom address post.
Before you pay
Read every field once more, because payment is the point of no return. Paid orders cannot be changed. If you spot a mistake before paying, abandon that order and place a new one with the corrected details. If several people are ordering together, the cart applies the tier price automatically, as the group orders guide explains. When everything matches, submit and move to payment, which is covered in the Bitcoin guide and the payment methods page. The last decision is shipping, and the Super Express guarantee post covers the only guaranteed tier.
Frequently Asked Questions
What happens if I make a mistake on the order form?
If you catch it before paying, place a new order with the corrected details. Paid orders cannot be changed, so the review step matters.
Can I leave the signature blank?
Yes. The signature is optional. Leave it blank and a clean signature is generated for you, so you are not forced to photograph handwriting.
Where do I put a custom address?
Type it into the extra info box at checkout. If you leave the box blank, a random, real address is printed on the card instead.
Does the date of birth have to be over 21?
On US state cards, yes. A younger date is automatically corrected to show over 21. Under-21 formats exist only for UK and Canadian cards.
Should I order my own state?
Ordering a neighbouring state is the practical choice, because door staff know their home state's card by heart. The template and scan quality are the same either way.
Can I change my order after paying?
No. Paid orders cannot be changed, which is why the form asks you to double-check every field before you submit payment.
